I'm not familiar with the history of the pinch collar, but I'll share anything I discover in my e-zine.
Regarding using the collar with one ring (the D-ring) or two (the D-ring and the safety ring), I typically attach the leash to the D-ring. This is the intended design. However, you can connect the leash to both rings to lessen the correction, which may be helpful for very sensitive dogs or specific training exercises.
For example, during protection work, we teach dogs to lunge only upon the attack command. If a dog lunges prematurely, a correction is given. In such cases, it's better to under-correct rather than risk the dog feeling insecure about lunging.
